Airway management principles

  • Anticipate analgesia and sedation before neuromuscular blockade wears off.
  • Review drug characteristics in the context of shock, bronchospasm, neurologic injury, agitation, organ function, age, and hemodynamics.
  • Verify published ranges, dose limits, concentration, route, monitoring, and contraindications against local protocol.

Medication characteristics to review

  • Fentanyl: analgesia; hypotension, respiratory depression, and chest-wall rigidity can occur.
  • Propofol: titratable hypnosis; substantial hypotension may occur.
  • Ketamine: analgesia and dissociation; still requires physiologic and hemodynamic judgment.
  • Midazolam: amnesia and sedation without analgesia; accumulation, hypotension, and delirium are concerns.
  • Dexmedetomidine: cooperative sedation or adjunct; slower onset, bradycardia, and hypotension are considerations.
